Set up your participant account

Please review the following information to help set up your participant account:

  1. Use the link provided in Step 6 below to reach the Survey Participant Portal. There, enter your email address under LOG IN and click “Forgot your password?”  Note that the email you use must be the same as the one where you received the invitation to participate in the survey.
  2. On the REQUEST PASSWORD RESET page, enter the same email address and click “Submit.” A password reset email (sender: no-reply@illinois.edu) will be sent to the email you provided.  Once you have received the email, click the link inside to reset your password.  This link will be valid for one hour.
  3. On the RESET PASSWORD page, enter your email address and a password; reconfirm your password and then click “Reset Password.”
  4. Once you have reset your password, click “return to your form” to log in with your email and password to reach the Participant Portal.
  5. Follow the instructions there to start the survey.
  6. Now click HERE to reach the Survey Participant Portal.
